What is Maggot Debridement Therapy (MDT) ?

Maggot debridement therapy (MDT) or bio-surgery in introduction of live disinfected maggots or maggot eggs into the chronic non-healing skin and soft tissue wounds for debridement, disinfection and cleaning of the necrotic tissue. MDT is an adjunct to Standard of care – managing diabetes, diet, protein supplements, wound dressing, and off-loading. MDT can help reduce manual debridement and dependance on antibiotics.

Selected Chrysomya megacephala (Bottle Green Housefly) species are bred, and their disinfected eggs or maggots are applied to specific wounds.

Indications & Contraindications for MDT in D.F.U

Indications:
MDT is useful for chronic diabetic foot ulcers (DFUs) that have necrotic tissue surrounded by healthy granulation, where surgical debridement is difficult. It is also indicated in cases with multi-drug resistant (MDR) bacterial infections.

Contraindications:
MDT should be avoided in patients with peripheral arterial disease or TAO with an ankle-brachial index below 0.8, those with chronic kidney disease (CKD), septicaemia, or multiple organ dysfunction syndrome (MODS). It is also not suitable for DFU patients without peripheral neuropathy or for those who are allergic to maggots or any components of MDT, though such allergies are very rare.

Debridement – removal of necrotic tissue

Maggots are voracious eaters, with the help of 2 strong chitinous hooks, they clean and digest tissue in the wounds.

Disinfection

Maggots secrete and excrete antibacterial compounds, when they feed on dead/necrotic tissue, the cocktail of secretions—containing maggot’s saliva, gut, and excretions—kills harmful bacteria residing in the wound bed.

Stimulation of Healing

Activates fibroblasts and movement of granulation tissue formation. Fibroblast stimulation = faster healing.

Biofilm Inhibition and Eradication

Scrapes, dissolves, and digests wound biofilm, minimizing drug and antibiotic resistance.
